Parents must fill out this permission form to allow Love Never Fails before events to take photos and videos of their children, as we will use it on our social media, website, and other promotional items. Please take the time to fill out this form. If parents opt their children out of getting photos or videos taken, we will ensure to respect the request and remove the child(ren) from any group photos or videos.

I consent for Love Never Fails to photograph or video my child and me or use photograph(s) or videos that already exist of my child and me taken during events or other functions. I understand that Love Never Fails may use photographs, digital images, or video segments in print or electronic media, websites, or social media pages owned and operated by Love Never Fails. I give Love Never Fails permission to publish, exhibit, and distribute these materials to publicize Love Never Fails programs/events or other lawful purposes without expectation of payment or further consideration.

By signing this consent form, I understand that:

• The materials are used only for nonprofit/educational purposes. 

• Love Never Fails will not release the materials to any other nonprofit or commercial entity without seeking my permission.

• Any proceeds from the sale of published or printed matter containing the materials will be used to support the mission of Love Never Fails. 

• The materials will become the property of Love Never Fails, stored in a place chosen by Love Never Fails, and not returned to me.

In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. I waive any right to royalties or other compensation related to using the materials. I now hold harmless and release, and forever discharge Love Never Fails from all claims, demands, liability, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons act on my behalf or on behalf of my estate have or may have because of this authorization.
